XLMD Discharge allows users from all disciplines to create and continuously update the patient’s discharge record during the patient’s length of stay through any internet-enabled computer or device. XLMD Discharge combines the discharge assessment, instructions, medication reconciliation, and patient teaching into a single, user-friendly view for expeditious patient processing, improved communications, and better bed management by the organization. At the conclusion of the patient’s stay, XLMD Discharge allows for a “patient-friendly” printout with all relevant patient discharge information organized in an easy-to-understand format, and a “chart-friendly” printout for the paper chart or electronic submission to the facility’s electronic repository and follow up physicians’ office systems.
